- License Application Form
- Company letter with applicant's name, addressed to the Chief of Training – EWA, stating the need the exam and confirming that the company has no objection to obtaining the licence (signed and stamped by the company owner or authorised representative)
- Copy of Qualification Certificate
- ID card reading data with updated printout date
- New clear photo (30 x 35 cm) with white background
- Copy of the CRPEP License
- Renew CRPEP license if close to expiration (attach receipt)
